What Is The Vision for Life Plan?

The Vision for Life LASIK enhancement plan is our ongoing commitment to preserving your best possible vision. Our vision correction surgeons are committed to ensuring the long-term stability of your results, which is why we proudly offer the Vision for Life Plan for LASIK and PRK recipients.

Should your vision change over time and a laser enhancement be deemed medically appropriate, patients who enroll in the Vision for Life Plan are eligible to receive the enhancement procedure from an American Vision Partners surgeon at no additional cost.

Who Is Eligible for the Vision for Life Plan?

You are eligible to enroll in the Vision for Life Plan when you undergo LASIK and PRK surgery performed by an American Vision Partners surgeon.

To maintain eligibility for this plan, you must get annual eye exams from an American Vision Partners provider or other qualified vision provider. You also need to provide documentation of annual eye exams and be in general good eye health to qualify for enhancement under the Vision for Life Plan.

If you need an enhancement procedure, it must be performed by any American Vision Partners provider. The Vision for Life Plan will not cover enhancement procedures performed by outside providers. 

The Vision for Life Plan is not available to:

  • ICL or RLE patients
  • Patients coming in for an enhancement following a surgery performed here or elsewhere
  • Patients seeking retroactive enrollment — the plan must be purchased at the time of the original surgery

Enrollment in the Vision for Life Plan

Eligible patients will have the opportunity to enroll in the Vision for Life Plan before receiving their PRK or LASIK laser vision correction procedure. There is a one-time enrollment fee of $275 per eye ($550 total) for the Vision for Life Plan. Your doctor will provide the paperwork and go over all the details with you before your procedure. Once you are fully informed about the details and eligibility requirements, you simply sign the Vision for Life Plan forms and pay the fee. There’s nothing else you need to do!

How the Vision for Life Plan Works

During your FREE LASIK or PRK consultation or before surgery, tell your vision correction team that you’re interested in choosing the Vision for Life Plan. Your care team will review the program, answer your questions, and have you sign the enrollment acknowledgment before surgery.

After surgery, follow your surgeon’s recovery instructions, use your prescribed medications, and attend all scheduled follow-up appointments during your first 12 months. This helps ensure the best possible healing and long-term results.

After your first year, have a comprehensive eye exam every year to maintain your eligibility. You may see a Barnet Dulaney Perkins provider or another American Vision Partners qualified eye care professional. Routine eye exams are not included with the Vision for Life Plan and are billed separately through your vision or medical insurance, or paid out of pocket if applicable.

To remain eligible for future enhancements, you must:

  • Complete your first year of postoperative care
  • Receive annual comprehensive eye exams
  • Keep documentation of those annual exams
  • Remain in good overall eye health
  • Receive any qualifying enhancement from an American Vision Partners surgeon

If you notice changes in your vision years after your procedure, schedule an evaluation with a Barnet Dulaney Perkins vision correction provider. Your original surgeon or another experienced refractive surgeon can assess your eyes and determine the cause of the change.

If your eye surgeon determines that a laser enhancement is medically appropriate and you continue to meet the Vision for Life eligibility requirements, your enhancement procedure is performed at no additional cost.
